Nurse Specialist Role

Are you a dedicated and compassionate healthcare professional seeking a challenging role? This position involves delivering annual health risk assessments in residential settings or via telehealth, working within the scope of practice for a Nurse Practitioner as delegated by a Collaborative Physician.

Key Responsibilities
  • Conduct comprehensive home health risk assessments to identify active and chronic disease conditions, as well as determine physical, mental, and social needs present at the time of the visit.
  • Take patient history, examine, and determine diagnoses.
  • Provide written documentation of patient visits, adhering to NCQA standards.
  • Take patient vital signs as necessary.
  • Place case management referrals and communicate with PCPs as needed.
  • Communicate with patients, caregivers, agency nurses, other providers, and vendors to ensure proper diagnosis.
  • Perform all clinical duties while observing OSHA Universal Precautions.
  • Maintain patient confidentiality.
  • Attend required meetings and in-services, participating in committees as requested.
Requirements
  • Active/unrestricted medical license.
  • Active CPR Certification.
  • Board eligible or board certified in family medicine.
Preferred Qualifications
  • Geriatric training/experience.
  • Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public.
Working Conditions
  • Ability to adapt to differing weather conditions and patients' home/residential environments.
  • Full range of body motion including handling/lifting patients, manual dexterity, eye-hand coordination, normal visual acuity, normal hearing, standing, bending, walking, and stair climbing.
  • Regular lifting/carrying items weighing up to 50 pounds.
  • Ability to ride in an automobile or van up to 150 miles daily in urban and/or rural settings.
