Restorative Care Professional

We are seeking a skilled and compassionate individual to join our team as a Restorative Care Professional.

This dynamic role involves coordinating and delivering restorative and rehabilitative care services to support optimal health outcomes for clients, promoting independence and functional recovery.

About the Role
  • Engage with community stakeholders to deliver culturally responsive, holistic, and inclusive care.
  • Coordinate assessments and develop support plans that cater to individual needs.
  • Provide ongoing support, advice, and guidance to ensure seamless care delivery.
  • Maintain quality care processes and manage risks associated with client care.
About You
  • Possess current registration as a Registered Nurse or Allied Health professional.
  • Hold specialist certifications/qualifications in relevant disciplines (highly regarded).
  • Demonstrate extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services.
  • Exhibit a strong understanding of reablement, functional rehabilitation, and person-centred care models.
  • Show proficiency in risk management, compliance, and quality assurance processes.
  • Demonstrate high-level communication, problem-solving, and decision-making abilities.
  • Display competence in using clinical documentation and care management systems.
  • Provide exceptional customer service, interpersonal, and communication skills, with the ability to deliver dynamic, person-centred service within the community and in clients' homes.
