We are seeking an innovative clinician to join our team and play a key role in supporting optimal health outcomes for clients. As a Restorative Care Clinician, you will be responsible for the clinical coordination and delivery of restorative and rehabilitative care services.
Key Responsibilities:
- Community engagement and stakeholder relationships
- Assessment and restorative care planning
- Ongoing support, guidance, and quality care processes
Requirements:
- Current registration as a Registered Nurse or Allied Health professional
- Specialist certifications/qualifications in relevant disciplines highly regarded
- Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
- A strong understanding of reablement, functional rehabilitation, and person-centred care models
- Proficiency in risk management, compliance, and quality assurance processes
- High-level communication, problem-solving, and decision-making skills
- Exceptional customer service, interpersonal, and communication skills with the ability to deliver person-centred service
- Proven experience working in a fast-paced human services environment managing high workloads and providing high-quality advice and support
