Role Description

Care Connect is seeking a dedicated Restorative Care Clinician to join our team. The successful candidate will be responsible for providing clinical coordination and delivery of restorative and rehabilitative care services in accordance with client support plans.

This role requires a high level of expertise in community care, aged care, disability, or community services, as well as excellent communication, problem-solving, and decision-making abilities.

The ideal candidate will have experience in clinical coordination, case management, or restorative care, and possess a strong understanding of reablement, functional rehabilitation, and person-centred care models.

  • Ongoing support, advice & guidance
  • Quality care processes & risk management
Requirements:
  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications / qualifications in relevant disciplines are highly regarded
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
  • Strong understanding of reablement, functional rehabilitation, and person-centred care models
  • Proficiency in risk management, compliance, and quality assurance processes
  • High-level communication, problem-solving, and decision-making abilities
  • Competence in using clinical documentation and care management systems
  • Exceptional customer service, interpersonal and communication (listening, written and verbal), and liaison skills
