Support Coordination in High-Risk Populations
The role of the Suicide Prevention Support Coordinator is to provide non-clinical care and individualized practical support to individuals and their families who have experienced a suicide attempt. This includes participation in intake and assessment processes, developing safety and support plans, and providing ongoing support during the first three months following a suicide attempt.
The coordinator will work closely with participants to connect them with essential services, promote social connections, and reduce the risk of future suicide attempts. Relevant qualifications or experience in working in a support role for people with mental health conditions are required. Additionally, experience in human services, individual and family support work, and intake and assessment is preferred.
Key Responsibilities:
- Participate in intake and assessment processes to identify participant needs
- Develop and implement safety and support plans with participants
- Provide ongoing support and coordination to participants during the first three months following a suicide attempt
- Connect participants with essential services and promote social connections
- Collaborate with multidisciplinary teams to ensure comprehensive care
Requirements:
- Relevant qualifications or commensurate experience in working in a support role for people with mental health conditions
- Experience in human services, individual and family support work, and intake and assessment
- Ability to work effectively with vulnerable populations and their families
- Strong communication and interpersonal skills
- Evidence of the Right to work in Australia
- COVID-19 Vaccination with booster or valid medical exemption
