A Career as a Care Coordinator at MyIntegra
We are seeking a dedicated and compassionate individual to join our team as a Support Coordinator. In this role, you will have the opportunity to work closely with our clients, understanding their unique needs and preferences. Your primary responsibility will be to assist NDIS participants in implementing their plans, coordinating services and supports, and ensuring seamless delivery.
Your Key Responsibilities
- NDIS Plan Implementation: Collaborate with clients to understand their goals and objectives, identifying relevant services and supports to achieve desired outcomes.
- Service Coordination: Develop and implement personalized service plans, ensuring effective coordination of care and support services.
- Client Relationships: Foster strong relationships with clients, families, and stakeholders, maintaining open communication and addressing concerns promptly.
- Administrative Tasks: Complete necessary paperwork and administrative tasks to ensure compliance with regulatory requirements.
Requirements and Qualifications
- Minimum Experience: At least 6 months to 1 year of experience working as an NDIS Support Coordinator.
- Certification and Education: Hold Allied Health qualifications or a minimum Certificate IV in Allied Health, Disability, Aged Care, or similar.
- Transferable Skills: Demonstrate excellent organizational, communication, and interpersonal skills, with the ability to work independently and as part of a team.
Compensation and Benefits
In recognition of your hard work and dedication, we offer a competitive salary of $ $60,000 - $80,000 per annum, depending on your level of experience.
