
Exceptional Client Support Specialist

The role of the HCP Care Partner is a dynamic and innovative position that focuses on delivering outstanding person-centred support to clients. Through building meaningful relationships with internal and external stakeholders, this role is responsible for ensuring that clients receive the care and support they need to live the life they choose.
- Coordinate services within established guidelines.
- Implement client service responsibilities in line with client needs.
- Ensure compliance with Aged Care Quality Standards and HCP requirements.
- Conduct assessments and reviews as required, and undertake intake of new clients.
This role involves coordinating services, implementing client service responsibilities, ensuring compliance with quality standards, and conducting assessments and reviews.
The HCP Care Partner will work closely with internal and external stakeholders to deliver exceptional client support and ensure that clients receive the care and support they need.
Requirements:To be successful in this role, you will have:
- Experience in a similar role and relevant qualifications.
- Knowledge, understanding, and experience in Aged care, disability, and/or Home and Community care.
- A genuine desire to deliver outstanding customer service in all interactions.
- Strong communication and interpersonal skills with a focus on building and fostering meaningful relationships.
