Senior Carer Role

About the Job

We are seeking an experienced Senior Personal Carer to join our team. The successful candidate will be responsible for conducting initial client intake and screening assessments, developing and reviewing individualised care plans, and providing quality care based on clients' needs.

The role involves:

  • Conducting thorough initial screening assessments and developing care plans for personal self-care, general house cleaning, social support, respite, transport, and shopping assistance.
  • Managing client information in CRM & CMS, ensuring all details and case notes are up-to-date.
  • Engaging in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team.
  • Spending time on the road completing scheduled visits, interacting one-on-one with clients within their homes, supporting daily activities.
  • Providing care in accordance with individualised Care Plans.
  • Supporting community-based recreational and social activities.
  • Transportation and assistance with shopping and appointments.
Requirements

To be considered for this role, you will need:

  • Certificate III Aged Care, Individual Support or an equivalent qualification - Essential.
  • Current Australian Driver's Licence (Green Provisional minimum) - Essential.
  • Roadworthy and insured (Third Party Property minimum) vehicle to use for work - Essential.
  • Experience with technology, including smartphones, tablets, laptops etc.
  • Confidence to satisfy relevant probity checks required by legislation or policies.
