About the Role
Aged Care Home Support Assessors play a crucial role in enhancing the quality of life for older Australians. They conduct in-person assessments, connect individuals with essential services, and work within a multi-disciplinary team.
Key Responsibilities:
- Conduct thorough assessments to identify individual needs and develop tailored support plans.
- Connect clients with local services, ensuring seamless access to necessary resources.
- Collaborate with healthcare professionals, family members, and other stakeholders to coordinate care.

Requirements
To be successful in this role, you will need:
- Experience in health, human services, community service coordination, or disability.
- Strong communication and interpersonal skills, with the ability to build trust and rapport with clients.
- Intermediate computer literacy, including proficiency in client management systems.
- A valid driver's license, a comprehensively insured vehicle, and willingness to travel.
