Job Opportunity

We are seeking a compassionate and skilled Cancer Navigator to join our team.

This role involves working with families impacted by cancer, providing high-quality support and connecting them to relevant services. You will work closely with family members to develop personalized plans, advocate for their needs, and coordinate the actions of multiple treatment and support providers.

About the Role:

Key Responsibilities:
  • Cancer Navigation Support: Provide evidence-based support to young people and families affected by cancer.
  • Psycho-Social Review Meetings: Actively participate in review meetings to communicate changes, challenges, and needs associated with each service user and their family.
  • Case Management: Utilize practical support methods to facilitate referrals and other supports, ensuring that families receive access to the best possible care.
  • Stakeholder Relationships: Identify and contribute to key stakeholder relationships to ensure that families receive comprehensive support.
  • Collaboration and Consultation: Collaborate and consult with peers, share practice through teaching and learning opportunities, and contribute to team projects.

You may be required to occasionally work outside of standard hours, but advance notice will be given. We have a flexible leave policy in place to accommodate your needs.

About You:

  • Experience: Demonstrated experience in delivering case management and/or clinical intervention within child, youth or health focused settings.
  • Tertiary Qualification: Tertiary qualification in Social Work is highly regarded.
  • Sector Partnerships: Experience creating and maintaining relationships with sector partners such as hospitals, health and community services, education and not-for-profit professionals.
  • Legislative Knowledge: Knowledge of child protection legislation and mandatory reporting.
  • Teamwork: Experience working in a multi-disciplinary team, liaising with staff from different departments (internal and external) and working within a remote team.
  • Decision Making: Demonstrated skills in decision making, problem solving, negotiation and crisis management.
  • Communication: Outstanding communication skills and a natural ability to connect.
