We are seeking a highly skilled and compassionate Care Manager to lead our clinical care team in a state-of-the-art, premium aged care facility.

Key Responsibilities:
  • Oversee the daily operations of the care facility, ensuring adherence to regulatory requirements and company standards.
  • Create and implement personalized care plans that address the unique needs of residents, collaborating with healthcare professionals and families.
  • Lead and mentor the care staff, offering guidance and opportunities for professional growth and development.
  • Efficiently manage budgets, resources, and supplies, maintaining a focus on cost-effectiveness and quality care.
  • Conduct regular assessments and audits to ensure that facility services consistently meet the highest standards.
Requirements:
  • Registered Nurse with a current AHPRA registration.
  • Proven experience as a Care Manager (or similar position).
  • Strong knowledge and understanding of the Aged Care accreditation standards.
  • Comprehensive knowledge and experience with ANNAC.
  • Strong leadership, communication, and interpersonal skills.
  • An outstanding proficiency in care planning, goal setting, and evaluation documentation.
  • Capability to effectively lead and manage a team.
