Job Title:
NDIS Care CoordinatorAllocate referrals efficiently to ensure timely and appropriate services for individuals with disabilities.
- Collaborate with stakeholders to convert enquiries into referrals while delivering exceptional customer service.
- Provide clear guidance and tailored advice on NDIS services and schemes.
- Work closely with operational teams and leaders to ensure a smooth intake process and positive service delivery experience.
- Manage funding requests to ensure available NDIS funds are utilized appropriately.
- Maintain accurate case management records, documenting all plans, progress, communications, and interactions.
Requirements:
Essential Skills and Qualifications:- Strong understanding of person-centred practice, NDIS principles, and disability service standards.
- Experience in intake, case management or similar client-facing roles.
- Excellent customer engagement, communication, and negotiation skills.
- Ability to work independently, manage priorities, meet targets, and adapt to changing environments.
- Proficient in Microsoft Office and database systems.
Additional Requirements:
- NDIS Worker Screening Clearance and Working with Children Check (or willing to obtain).
