Care Coordinator for Disability Employment

We are seeking an experienced Care Coordinator to join our Work and Study program at North Parramatta. The successful candidate will have a strong commitment to empowering people with disabilities to build and search for careers.

The key duties of this role include:

  • Assisting customers with employment goals, networking with employers on their behalf, and organizing work placements
  • Empowering support staff to establish effective communication with customers using individualized approaches
  • Implementing and reviewing Individual Program Plans
  • Recording customer progress according to relevant documentation
  • Developing and facilitating program activities and workshops with customers and support staff

Essential criteria for this role include:

  • Experience supporting customers with disabilities and assisting them to find jobs
  • Demonstrated commitment to supporting and promoting individual needs
  • Demonstrated ability to develop positive relationships with customers and support their care plans
  • A current Driver's Licence and confidence to drive an accessible van
  • NSW Working With Children Check or willingness to obtain
  • NDIS Worker Check or willingness to obtain
  • Current First Aid and CPR certificate or willingness to obtain
  • Excellent verbal and written communication skills

This position offers a competitive salary package, including not-for-profit salary packaging and a reward and recognition program.

Estimated salary: $65,000 - $80,000 per annum, depending on experience.
