Care Coordinator

The Care Coordinator is a key member of our Salvos Home Care team, responsible for the ongoing coordination and management of services and supports for a group of Home Care Package or CHSP clients. This involves building strong partnerships with clients, their families, and the Salvos Home Care team. The Care Coordinator is supported by the Operations Manager and a stable team, and is part of our broader Aged Care Support Services.
- Conduct initial assessments and onboarding for new clients, and maintain ongoing assessments and relationships with clients and their representatives.
- Develop, implement, and monitor consumer-directed care plans in collaboration with clients.
- Provide consumer-directed case management services to clients, acting as the main point of contact between clients, their informal carers, Home Care Workers, and external service providers.
- Establish and maintain positive working relationships with all members of the Salvos Home Care team and the broader Aged Care Support Services team.
- You will have an appreciation of and the ability to promote, support, and model the Christian values and ethos of The Salvation Army.
- You will possess tertiary qualifications in a relevant field or a minimum of three years industry experience.
- You will be motivated to work with older people living in the community and have an understanding of consumer-directed care and enablement approaches.
- You will have proven leadership experience in overseeing, coordinating, and implementing consumer-directed care services within a Home Care setting.
- Excellent oral and written interpersonal skills, strong customer service focus, and ability to negotiate and influence others.
- You will have a current driver's license, good driving record, and willingness to travel for work.
