Home Care Coordinator Specialist

About The Role
The Salvation Army is seeking a dedicated and compassionate Home Care Coordinator to join our team. As a key member of our Salvos Home Care team, you will play a vital role in providing high-quality support services to our clients.
As a Home Care Coordinator, you will be responsible for coordinating and managing a comprehensive range of services and supports provided to our clients. This will involve building strong relationships with clients and their families, as well as collaborating with our team to ensure seamless delivery of care.
Key Responsibilities
- Undertake initial assessments and onboarding for new clients, maintaining ongoing assessments and exemplary relationships with clients and their representatives.
- Develop, implement, and monitor consumer-directed care plans in collaboration with clients.
- Provide consumer-directed case management services to clients, acting as the main point of contact between clients, their informal carers, Home Care Workers, and external service providers.
- Establish and maintain positive working relationships with all members of the Salvos Home Care team and with the broader Aged Care Support Services team.
About You
To be successful in this role, you will have:
- An appreciation of and ability to promote, support, and model the Christian values and ethos of The Salvation Army.
- Tertiary qualifications in a relevant field or minimum of three years industry experience.
- Motivation to work with older people living in the community and an understanding of consumer-directed care and enablement approaches.
- Proven leadership experience in overseeing, coordinating, and implementing consumer-directed care services within a Home Care setting.
- Excellent oral and written interpersonal skills, strong customer service focus, and ability to negotiate and influence others.
- A current driver's license, good driving record, and willingness to travel for work.

This is an exciting opportunity to join a dynamic and supportive team, working towards making a positive impact on the lives of our clients. We offer a competitive salary package of approximately $70,000 - $90,000 per annum, dependent on experience and qualifications.
