Job Description

We deliver acute care in the patient's own home as part of our innovative Hospital in the Home service.

Our multidisciplinary teams work collaboratively to provide comprehensive care and support to patients, enabling them to recover at home with their loved ones by their side.

This rewarding role requires you to use the HITH model of care to deliver high-quality treatment and support to patients in their homes.

  • You will be part of a close-knit team that shares a passion for delivering exceptional care.
  • Work closely with nursing, medical, physiotherapy, occupational therapy, pharmacy and social work colleagues to ensure seamless care delivery.

The role is based at Royal North Shore Hospital, one of our main bases across the Northern Sydney Local Health District. This provides opportunities to work with diverse communities and make a positive impact on people's lives.
