As an occupational therapist in our multidisciplinary team, you will conduct comprehensive aged care assessments and provide referrals, support, and information to patients and their families.

You will play a crucial role in ensuring efficient, effective, and timely assessments for clients in both acute care facilities and community settings.

This role promotes uniformity and equity of access to Australian government-funded aged care services.

Aged care assessment training is provided through the My Aged Care learning portal.

We seek a skilled occupational therapist to join our team to conduct assessments and develop referral pathways for patients requiring care.

In this role, you will work closely with other healthcare professionals to ensure that patients receive the best possible care.

Your primary responsibility will be to assess patients' needs and develop individualized plans to meet those needs.

The successful candidate will have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

They will also possess strong analytical and problem-solving skills, as well as the ability to work independently and as part of a team.

You will report to the manager of our aged care assessment team and will be responsible for collaborating with other healthcare professionals to identify and address the needs of patients.

This role requires a high level of professionalism, discretion, and confidentiality at all times.
