About the Role
We are seeking a dedicated and compassionate Support Worker to join our team at Activ. As a Support Worker, you will play a vital role in providing high-quality support to our customers, enabling them to live independently and achieve their goals.
Key Responsibilities
- Provide individualized support to customers, promoting their independence and well-being
- Develop and implement personalized support plans, working collaboratively with customers, families, and other stakeholders
- Contribute to a positive and inclusive team environment, promoting a culture of respect and empathy
- Participate in ongoing training and professional development, ensuring expertise in disability support and NDIS practices
- Collaborate with other Activ teams, including RTO and HR, to ensure seamless service delivery

- NDIS Worker Screening Check
