
Aged Care Specialist

This role provides an opportunity to work in a dynamic team providing aged care assessment services across the Northern Sydney Local Health District. As a needs assessor, you will be responsible for conducting comprehensive assessments of older people to identify their care needs and developing individualized care plans.
- Key Responsibilities:
- Main duties include:
- Conducting home visits or hospital assessments to evaluate the health and functional status of older people
- Developing and implementing individualized care plans that address the identified needs of clients
- Liaising with healthcare professionals, family members, and carers to ensure coordination of care
- Providing education and support to clients and their families to promote self-management and independence
- Essential Skills and Qualifications:
- A relevant tertiary qualification and registration as an allied health professional or registered nurse are required.
- A proven track record of working in aged care settings and experience in assessing and supporting older adults is essential
- Demonstrated expertise in using technology, including electronic patient record systems and e-learning platforms
- Strong written and verbal communication skills, with the ability to produce high-quality reports and presentations
