About the Role:
The Social Worker will be accountable for providing quality patient care through comprehensive psychosocial assessments, development of intervention plans and collaborative practice with the Multi-disciplinary team.
The position is located at both our Broadmeadows and Craigieburn Campuses.
This position requires the successful applicant to engage with and work efficiently with patients, family/carers, multidisciplinary staff and external providers to develop and implement safe plans to support patients to remain in the community and prevent unnecessary hospital admissions.
The role requires the Social Worker to respond to all referrals in our community therapy service and outpatient clinics including the Cognitive Assessment and Dementia Management service (CADMS).
The Social Worker will be responsible, on a rotating roster with other multidisciplinary team members, for facilitating the cardiac rehab group.
It will be beneficial for the Social Worker to have significant experience and skills working with patients & the families of those who have dementia, as well as clients requiring assistance with navigating the National Disability Insurance Scheme (NDIS), Victorian Civil and Administrative Tribunal (VCAT) and My Aged Care (MAC).
This role will require flexibility to be able to provide an efficient response to referrals combined with other non-clinical components such as providing education to staff, clinical supervision of Gr 1 staff and Social Work students, innovation and quality improvement, actively participating in relevant meetings, and contributing as a member of the Sub Acute Social Work team.
Staff will be required to travel and/or work across the sites of Northern Health. Staff may also be required to rotate to other campuses and rotate into other grade-appropriate discipline positions.
Key Responsibilities:
- Responding to referrals to Social Work.
- Participating in a Quality Improvement activity.
- Completing all data sets.
- Supervising Gr 1 Social Workers and Social Work students.
- Contributing to Social Work initiatives and engaging in Quality Improvement Activities.
- Reporting and feedback as per NDIS policy and mandatory reporting timeframes.
Requirements:
- Significant experience in undertaking comprehensive psychosocial assessments & interventions.
- Significant experience in practicing patient centred care.
- Demonstrated experience in prioritising and independently managing complex social situations.
- Demonstrated commitment shown to ongoing professional development.
- Dedication and demonstrated involvement to Quality Improvement.
- Experience in education, support and supervision to Grade 1 Social Workers and/or students.
- Experience working with the cultural and socioeconomic diversity of our community.
- Extensive knowledge of relevant community services relative to health social work.
- Knowledge of processes, legislation and community services related to Child Protection, Family Violence, VCAT, Aged Care, NDIS, Elder Abuse, Homelessness and Mental Health.
- Skilled at safety planning and managing risk, especially for survivors of Family Violence.
- Competent computer skills particularly in word processing and internet use and the ability to learn dedicated software.
