About the Role

CareChoice is seeking a highly skilled and compassionate Disability Support Worker to join our team in the McMahons Creek region. As a Disability Support Worker, you will play a vital role in providing quality support to participants with complex needs and behaviors of concern.

Key Responsibilities
  • Provide person-centered support to participants in their own homes and purpose-built facilities
  • Develop and maintain professional relationships with participants, their families, and multidisciplinary teams
  • Collaborate with colleagues to ensure seamless delivery of services and achieve exceptional outcomes
  • Contribute to the development and implementation of individualized support plans
  • Participate in ongoing training and professional development to enhance skills and knowledge
Requirements
  • Certificate 4 level qualification in a relevant field (e.g., Disability, Community Services, or a related field)
  • Clear and effective communication skills, with the ability to establish professional relationships
  • Attention to detail and accuracy in record management
  • Flexibility to work over a 7-day roster, including overnight shifts and training/meetings as required
  • NDIS Worker Screening Clearance or willingness to obtain
  • First Aid, CPR, WWCC, and NDIS Worker Orientation module
  • Current Driver's License with access to a comprehensively insured vehicle (mandatory)
