About the Job:

  • To continuously monitor, review, and evaluate care plans, services, and outcomes.">
  • To assist clients in navigating the care system and linking them to necessary resources where required.">

Requirements:

  • A current AHPRA registration and unlimited work entitlements.">
  • Demonstrated understanding and sound knowledge of client-centred, community-based care services delivery.">
  • Excellent communication and clinical problem-solving skills.">
  • A current Australian Driver's Licence and a roadworthy vehicle for work purposes.">
  • Confidence in managing your time effectively, including an ability to work autonomously.">
