Job Title: Home Care Service Coordinator

We are seeking an experienced and skilled individual to coordinate our home care services, ensuring efficient and effective delivery of client services to our community support workers.

This is a key role in our team, requiring strong administrative skills and exceptional customer service delivery. The successful applicant will have advanced-level experience as a scheduler and be familiar with client management systems such as Comcare/and or E-case.

About the Role:

  • Evaluate client requirements and schedule home care staff to meet these needs
  • Schedule services to maximize utilization of staff in line with their availability, location, and training competencies
  • Ensure all staff and client communication is recorded in the Aged Care Management System
  • Provide exceptional customer service through regular phone calls and emails
  • Attend and participate in scheduling team training sessions and meetings
  • Collaborate with external agencies to request service coverage where required

Requirements:

  • Advanced-level experience as a scheduler
  • Familiarity with client management systems such as Comcare/and or E-case
  • Excellent organizational and time management skills
  • Ability to work independently with minimal supervision
  • Strong communication and interpersonal skills
