We are seeking a dedicated and compassionate individual to work with clients and their families in their homes, enabling them to maintain their independence.

Your Key Responsibilities Will Include:

  • Providing support to clients in their community, who have been referred for assistance at home
  • Developing and implementing services and programs that meet the unique needs of each client
  • Fostering strong relationships with families and carers, acknowledging their individual requirements
  • Working closely with clients to set achievable goals, taking into account their strengths and challenges
  • Adopting best practice approaches and collaborating with a multidisciplinary team to deliver high-quality care
  • Maintaining accurate records using client management systems

What We Need From You:

  • Ability to work independently in the community
  • Excellent verbal communication skills
  • A patient, empathetic, and caring nature

Requirements:

  • Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management) or equivalent knowledge and experience
  • Experience in aged care or transferable skills will be highly regarded
  • Demonstrated ability to work within established guidelines and achieve service delivery objectives
  • A valid driver's licence
  • Intermediate level of computer skills
  • Effective time management and organisational skills
  • Ability to work collaboratively as part of a team
