About the Position:
We are seeking a dedicated and compassionate individual to work with clients and their families in their homes, enabling them to maintain their independence.
Your Key Responsibilities Will Include:
- Providing support to clients in their community, who have been referred for assistance at home
- Developing and implementing services and programs that meet the unique needs of each client
- Fostering strong relationships with families and carers, acknowledging their individual requirements
- Working closely with clients to set achievable goals, taking into account their strengths and challenges
- Adopting best practice approaches and collaborating with a multidisciplinary team to deliver high-quality care
- Maintaining accurate records using client management systems
What We Need From You:
- Ability to work independently in the community
- Excellent verbal communication skills
- A patient, empathetic, and caring nature
Requirements:
- Bachelor's Degree or Diploma of Community Services (Service Coordination), or Diploma of Community Services (Case Management) or equivalent knowledge and experience
- Experience in aged care or transferable skills will be highly regarded
- Demonstrated ability to work within established guidelines and achieve service delivery objectives
- A valid driver's licence
- Intermediate level of computer skills
- Effective time management and organisational skills
- Ability to work collaboratively as part of a team
