Mental Health Care Coordinator

We are seeking a passionate and organised Registered Nurse to join our dynamic Specialist Community Mental Health Service.
Key Responsibilities- Deliver comprehensive clinical care coordination, facilitating communication and collaboration between patients, clinicians, and community stakeholders.
- Develop diverse professional portfolios, including experience in depot and clozapine clinics, supporting consumer fitness groups, and facilitating therapeutic interventions.
- Conduct thorough assessments, utilising expertise to perform mental health and psychosocial assessments, risk assessments, and develop personalised treatment plans.
- Support team leadership, collaborating with the Team Manager to contribute to a cohesive and efficient team environment.
- Contribute to quality improvement, engaging in clinical auditing, research, and evaluation initiatives to continuously improve patient care outcomes and service delivery.
- You hold a current practicing certificate as a Registered Nurse with the Nursing and Midwifery Board of Australia.
- You have demonstrated clinical expertise and experience in mental health services.
- You possess strong communication, collaboration, and leadership skills.
- You are committed to continuous professional development and evidence-based practice.
- A generous annual salary of $96,786 - $123,511 (pro rata), based on the Southern Adelaide Local Health Network's salary scale.
