Uniting Communities is a leading not-for-profit organisation providing holistic and innovative alcohol and other drug services.

We offer 1:1 counselling, group work, case management and residential rehabilitation across our service. We are seeking motivated professionals to join our team in Adelaide Metro rehabilitation services.

The Counsellor/Case Manager role includes:

  • Office and outreach based therapeutic AOD services for residential client groups, outpatient counselling groups and police drug diversion services
  • Supporting individuals accessing AOD residential programs to make long-term changes in their lives
  • Developing partnerships with the community to create better outcomes for clients and drive referrals into the service
  • Breaking down stigma associated with AOD and mental health issues

Ideally you will possess a tertiary qualification in Social Work, Psychology or related field, plus experience. You will have demonstrated professional experience in providing intensive support, counselling and case management, including working with internal staff and external stakeholders.

You will have strong ability to build and maintain positive relationships with diverse clients, community groups and agencies. Uniting Communities values diversity and provides respectful, accessible services.

Key Responsibilities

Service Delivery:

  • Lead residential therapy sessions and facilitate case management plans to empower clients towards recovery
  • Establish and maintain collaborative relationships with local healthcare providers, community organizations and law enforcement agencies
  • Conduct thorough assessments and develop individualized treatment plans that cater to diverse needs

Professional Development:

  • Pursue ongoing education and training to enhance your expertise in AOD treatment and crisis intervention
  • Stay updated on industry best practices and research-based methodologies to inform your practice

Collaboration and Partnerships:

  • Foster strong alliances with community leaders, service providers and government agencies to promote collective impact and resource sharing
  • Develop and implement referral pathways to ensure seamless transitions between services and optimize client outcomes
