Senior Care Needs Assessor

We are seeking a dedicated and compassionate Senior Care Needs Assessor to join our team.

In this critical role, you will play a pivotal part in identifying the needs of older adults and connecting them with essential services that support independent living.

Your key responsibilities will include:

  • Conducting thorough assessments to identify the care needs of older adults.
  • Facilitating referrals to vital services that promote self-sufficiency and dignity.
  • Actively promoting the retention and maximization of function and capability in daily living.
  • Engaging with families and carers as key stakeholders.
  • Ensuring all assessments are conducted in line with relevant contractual requirements.

To be successful in this role, you will require:

  • A minimum of 1 year's experience in aged care, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline (e.g. nursing, occupational therapy, physiotherapy, social work).
  • Current unrestricted General Registration with a relevant professional association.
  • Proficiency in Microsoft Office and client management systems.
  • A current Australian driver's licence, comprehensively insured vehicle, and the ability to travel within your designated service area.
