Job Title: Skilled Healthcare Provider

The role of the Skilled Healthcare Provider is to deliver high-quality patient care in a dynamic and supportive environment. As a Skilled Healthcare Provider, you will have the opportunity to work with a diverse range of patients and families, developing your skills and expertise in a rapidly changing healthcare landscape.

  • Develop and implement individualised nursing care plans for patients, ensuring that their physical, emotional, and psychological needs are met.
  • Conduct thorough assessments, identifying potential risks and opportunities for improvement, and develop strategies to address these issues.
  • Collaborate with multidisciplinary teams to achieve patient-centred outcomes, including collaborating with medical staff, allied health professionals, and other healthcare providers.
  • Maintain accurate and comprehensive patient records, including documentation of medication administration, vital signs, and other relevant information.
  • Communicate effectively with patients, families, and healthcare providers, using clear and concise language to ensure that everyone is informed and involved in the care process.

Responsibilities include:

  • Healthcare assessment and planning
  • Administration of medications and treatments
  • Monitoring of patients' conditions and response to treatment
  • Documentation of patient care and progress
  • Collaboration with multidisciplinary teams to achieve patient-centred outcomes

Requirements include:

  • Current registration as a Registered Nurse with AHPRA
  • National Criminal History Check
  • Specialist qualifications and/or experience as appropriate to the area of work
  • A valid working with children check
