Job Title

We are seeking a Support Professional to join our team on a full-time basis.

The role will involve managing and supporting the transition of hospital patients with a disability to suitable accommodation and funding.

The key responsibilities include:

  • Transition management for hospital patients with a disability
  • Rapport building and individual support needs assessment
  • Cross-functional coordination with dedicated support workers, administration, clinical & allied health support, and unit management

The ideal candidate will hold a minimum cert 4 in Community Services/Disability/Mental Health or alike, NDIS Worker Screening Check, NDIS Worker Orientation Module _Quality, Safety and You_, evidence of Influenza vaccination, and full working rights in Australia.
