About the Role:

The State Heartprint Lead is a key position within St Vincent's Health Australia, responsible for ensuring the successful implementation and embedding of the Heartprint model across all facilities in Queensland.

Key Responsibilities:

  • Be the primary point of contact and accountability for the Heartprint Roll-out in QLD.
  • Support facilities to lead site-based Heartprint Champions.
  • Work closely with Heartprint Champions to provide guidance and support on challenges related to the model's interpretation and delivery.
  • Support with site-based Orientation on Heartprint education and implementation.
  • Support facility managers, clinical managers, and Living Support Managers on care-related outcomes to identify opportunities for improvement.
  • Contribute to quality and compliance KPIs with facility leaders regarding identified trends and risks.

Requirements:

  • Two plus years of Aged Care Leadership or Management experience.
  • Experience in an aged care environment.
  • Understanding of Aged Care and resident rights and responsibilities.
  • NDIS Worker Screening Check may be a requirement of this role.
  • Complete Key Personnel Checks (including Australian Securities and Investments Commission and the Australian Financial Security Authority checks).
