Job Opportunity: Healthcare Professional
We are seeking a skilled and compassionate Healthcare Professional to join our team.
- This role involves providing high-quality medical, psychiatric, substance abuse, and nursing care for individuals in need.
- The successful candidate will also serve as a clinical liaison, working closely with community health, mental health, and social service agencies to ensure seamless service delivery.
Key Responsibilities:
Service Coordination- Coordinate services with public and private healthcare providers to ensure access to necessary care.
- Assist individuals with the preparation and recording of their service plans.
- Conduct comprehensive assessments and develop individualized treatment plans.
- Provide counseling services to individuals and families.
- Supervise and/or provide nursing care, including monitoring vital signs such as blood pressure and diabetes management.
- Graduation from an accredited Nursing program is required; a Bachelor's Degree in Nursing is preferred.
- A valid and current state-specific Nursing license is mandatory.
- Minimum one year of experience working in a behavioral health setting is desired.
