
Meaningful Connections Coordinator

The role of a Home Companion is to establish and nurture meaningful relationships with residents in a unique residential community for individuals living with dementia.
This community provides a home-like environment that enables residents to participate in everyday household activities, including grocery shopping, meal preparation, and housekeeping.
As a Home Companion, you will create an environment that encourages residents to maintain their personal identity while feeling empowered and independent in their daily lives.
Residents have the freedom to choose how they live, spend their time, and undertake activities such as meals or personal care, supported by Home Companions as needed.
Meals are prepared fresh within each residence by the residents and the Home Companion. Home Companions work independently, empowering villagers to undertake activities of daily living and assisting and supporting them with normal daily tasks as required.
You will collaborate with the Health and Wellness Team and the Dementia Support Consultant to provide high-quality care and ensure our residents live their best life.
Key Responsibilities:
- Establishing and nurturing meaningful relationships with residents
- Creating an environment that encourages residents to maintain their personal identity
- Supporting residents with daily tasks such as meal preparation, household cleaning, and laundry
- Collaborating with the Health and Wellness Team and the Dementia Support Consultant
The ideal candidate will have a Certificate III in Individual Support/Aged Care/Community Services and experience working with aged people living with dementia or experience working with aged people or NDIS clients within their own homes.

Experience working with aged people living with dementia or experience working with aged people or NDIS clients within their own homes (Preferred)
Willingness to learn about various forms of dementia and how best to support residents
