Job SummaryWe are seeking a passionate and dedicated Care Coordinator to join our team in the Sydney Lower North Shore office. As a Care Coordinator, you will play a vital role in coordinating in-home care services, working closely with clients, care staff, and other stakeholders to ensure seamless delivery of care.
Key Responsibilities- Conduct thorough client care assessments and develop personalized care plans
- Supervise and manage care staff, providing guidance and support to ensure high-quality care delivery
- Engage with potential clients and referral sources, promoting Right at Home's services and building strong relationships
- Registered Nurse with a strong understanding of the principles of My Aged Care and Aged Care Reforms
- Significant experience in the Australian Aged Care or Disability sector, with a focus on culturally and linguistically diverse backgrounds
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, staff, and stakeholders
- Strong organizational and administrative skills, with the ability to prioritize tasks and manage multiple projects
- Comprehensive assessment and case management experience, with a focus on client-centered care
- Sound understanding of consumer-directed care and its application in the aged care sector
- Experience in managing staff and providing guidance and support to ensure high-quality care delivery
- Ability to work independently and as part of a team, with a strong focus on collaboration and communication
- Highly developed organizational, administrative, business, people, and financial management skills
