Case Manager Job Description

The role of the Case Manager involves working closely with individuals or families to assist them in obtaining and maintaining safe and stable long-term housing. This is achieved through comprehensive needs assessments and effective support planning, providing practical assistance where necessary, and connecting individuals and family members with a multidisciplinary team or appropriate additional support services.

This case management position requires a clear understanding of Housing First principles and the ability to provide flexible, trauma-informed, and culturally safe support to vulnerable community members with multiple and complex needs.

Key responsibilities include:

  • Conducting thorough needs assessments to identify individual and family requirements
  • Developing and implementing effective support plans tailored to each client's needs
  • Providing practical assistance and connecting clients with relevant support services

Selection Criteria:

To be successful in this role, you will need:

  • A relevant tertiary qualification in Social Work, Welfare, or Behavioural Science, or proven relevant experience
  • Knowledge and understanding of the Housing First Principles
  • Experience in providing holistic, strength-based, trauma-informed case management support to vulnerable community members
  • An ability to assess individual needs in a family context and develop, evaluate, and update case support plans using trauma-informed practice
