Job Title: Care Service Coordinator

We are seeking experienced professionals to work with our clients in Shepparton. As a Care Service Coordinator, you will be responsible for managing care plans and ensuring the best possible outcomes for our clients.

Key Responsibilities:

  • Manage care plans and coordinate services
  • Conduct regular assessments and reviews of client care plans
  • Develop and implement individualized care plans
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
  • Ensure compliance with relevant laws and regulations

Requirements:

  • Recent experience as a Case Manager of Aged Care Home Care Packages
  • National police check less than 12 months old
  • Current Working with Children's Check
  • Minimum of 3 COVID vaccinations

What We Offer:

  • Casual pay rate of $63.67 per hour plus 12% Superannuation
