About the Role:

This is an exciting opportunity to work in a purpose-driven team focused on delivering outstanding service to NDIS participants. The successful candidate will be responsible for assisting participants, their families, and carers in identifying needs through assessment and person-centred planning. They will also evaluate, develop, and deliver evidence-based support plans within a complex support space, ensuring individual plans reflect the participant's goals, passions, and strengths.

Key Responsibilities:
  • Assessing and developing person-centred plans that meet the needs of NDIS participants.
  • Evaluating and delivering evidence-based support plans within complex support spaces.
  • Developing meaningful goals and practical strategies using a positive behaviour support framework.
  • Collaborating with multidisciplinary teams to ensure high-quality support services.
  • Acting as an advocate and role model for best practice.

Requirements:

To be considered suitable, you will need:
  • Current suitability at the Proficient level as a Behaviour Support Practitioner, confirmed by the NDIS Quality and Safeguards Commission.
  • A degree-level qualification as an allied health professional.
  • Experience in NDIS report writing.
  • Excellent time management and organisational skills to manage a varied caseload.
  • A proactive attitude, motivated by improvement and a passion for working in the disability sector.
