Job Title: Care Coordination Specialist

As a Care Coordination Specialist, you will play a vital role in providing high-quality care to residents. You will work closely with the Clinical Care Coordinator and Registered Nurse to ensure individual needs, wishes, and goals are met in a supportive and dignified environment.

  • Key Responsibilities:
  • Provide social and emotional support to residents, ensuring holistic and person-centred care.
  • Coordinate care needs, including medication management, nutrition, and hydration, and pressure care management.
  • Lead and supervise care on shifts within designated wings or areas.
  • Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and policies.
Requirements
  • Qualifications:
  • Diploma of Enrolled Nursing (with APRHA Registration to administer medication), Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines), or Certificate III in Individual Support with willingness to undertake training to obtain required qualifications.
Personal Qualities:High level of commitment, integrity, honesty, and trustworthiness.Effective listener with ability to communicate at all levels, both orally and in writing.Ability to understand and use information technology and programs and to learn new skills as required.Current National police check.Working knowledge of WH&S and Infection Control.What We Offer
