Job Title

We are seeking a self-motivated Service Coordinator to join our team who can work autonomously as well as a supportive team member.

As an integral part of our team, you will possess the ability to meet deadlines and provide a responsive service to internal and external customers, providing high quality coordination of low level Home Care services.

This role is perfect for an Enrolled Nurse or Registered Nurse looking to move away from clinical patient care and get their weekends back.

Key Responsibilities
  • Provide quality low level Home Care services to customers through assessment and coordination.
  • Assist customers to navigate the aged care system, ensuring they receive the best possible range of care to meet their needs.
  • Encourage customers to remain independent and be an active participant in their community.
  • Actively promote AnglicareSA Home Care services in the community.
Requirements
  • Tertiary qualification in Community or Human Services or a health-related discipline
  • Current Australian Driver's Licence
  • Proven experience in a service coordination or advisory role
  • Demonstrated understanding of the needs of older people living in the community
  • Proven customer service, administrative and organisational skills, and ability to manage KPIs
  • Experience within Community Aged Care is desirable
