We are seeking an experienced and motivated Aged Care Specialist Coordinator to join our Homecare team in the South Metro region, servicing areas from south of the river to Mandurah and suburbs in between. This role will involve travel between all Bethanie sites and services to ensure an active approach to meeting the Homecare Service needs.

This permanent full-time role offers a salary range of $80,000 - $110,000 per annum, plus a travel allowance of $0.92c per kilometre. The ideal candidate will have current knowledge of the Aged Care Standards and Homecare Case Management principles. They will be comfortable with both the principles of case management and the ability to pivot and manage multiple priorities to achieve positive outcomes for new and existing recipients of Bethanie Homecare services.

About the Role
  • Oversee and coordinate the onboarding process of new Home Care Package clients for the assigned region.
  • Develop ongoing relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for the client’s health.
  • Assist clients in identifying ongoing needs and goals in order to support them to remain in their own home.
  • Responsible for ensuring all client budgets are monitored and managed in line with approved package level and individual client requirements.
  • Be an effective member of the multi-disciplinary team to ensure all decisions and referrals for clients deliver a high standard of holistic care.
About You
  • You will be willing to uphold the Mission, Vision, and Values of Bethanie.
  • You will have experience in a homecare case management environment either with a clinical background or more than 3 years as a Case Manager in a similar organisation.
  • You will have knowledge and experience within a person-centred care context within the Aged Care industry.
  • You will have knowledge of Case Management principles, Home Care Package Guidelines, and Funding body.
  • You will have experience developing and managing client budgets.
  • You will have the ability to work within a team, as well as autonomously.
  • You will have excellent organisational skills and ability to manage multiple priorities.
  • You will have excellent verbal and written communication skills and strong computer skills and experience with client database systems.
  • You will have a valid driver’s licence and access to a reliable vehicle.
  • You will have a current National Police Clearance, or be willing to apply for one.
  • You will be willing to obtain required vaccinations – COVID 19 (minimum of 3 doses) and annual flu vaccination.
