About the Role

The Child Health Outreach Program (CHOP) Team Leader plays a pivotal role in managing and coordinating the CHOP, ensuring cultural sensitivity and high standards of clinical care and case management. This position emphasizes outreach work with clinical, liaison, and administrative responsibilities.

Key Responsibilities
  • Providing leadership and coordination for the CHOP team to ensure seamless delivery of services
  • Ensuring cultural integrity and high standards of clinical care and case management
  • Developing and implementing strategies to improve community engagement and health outcomes
Requirements
  • Current registration as a Registered Nurse or Aboriginal Health Practitioner with AHPRA
  • At least five years' clinical experience
  • Demonstrated ability to lead and manage a multidisciplinary team
  • Experience in community-based child health nursing, case management, and chronic disease management
