Job Overview

The role of Intakes and Transitions Manager is to support residents in transitioning into their new homes, ensuring a seamless experience.

This position plays a vital part in supporting residents with dignity, confidence, and happiness in their home for life.

Key Responsibilities
  • Transition Management
    • Manage the participant journey from initial enquiry through to tenancy.
    • Ensure residents have a smooth transition into their new home.
  • Community Engagement
    • Conduct initial assessments, reviewing participant plans & profiles and verifying eligibility & funding.
    • Work alongside the Tenancy & Asset Management team to conduct regular site visits and tours with potential participants and stakeholders.
    • Maximise CRM efficiency to optimise contact strategies and gain customer and business insights.
  • Tenancy Retention
    • Support the tenant retention strategy, including newsletters, mini strata, discounts, events and other engagement initiatives.
Candidate Profile
  • We are looking for someone who is collaborative, proactive, and thrives in a dynamic environment.
  • You should be solutions-focused, resourceful under pressure and motivated to deliver outstanding results.
  • Ability to foster positive relationships with people from diverse backgrounds.
  • Strong customer relationship and stakeholder management skills.
  • Minimum three years demonstrated experience in transitions or business growth and development.
  • Solution-focused with well-developed problem-solving skills.
  • Outstanding written and verbal communication and influencing skills.
  • Values that align with our organisation.
  • Working knowledge of CRM systems and Microsoft Office suite.
  • Current NSW Drivers Licence and a willingness to travel to sites across Sydney, Central West, Central Coast, and the Illawarra region.
  • Relevant NDIS screening checks maintained and current.

Reporting Line: Head of Growth & Partnerships

Location: Based at our office. You will also be required to spend time at our community housing sites.
