About the Role

We are seeking a highly skilled and compassionate Specialist Aftercare Coach to join our team at Uniting. As a Specialist Aftercare Coach, you will play a vital role in supporting care leavers aged 18 to 24 in their transition to adulthood.

Key Responsibilities
  • Provide casework, coordination, and access to services to help young people achieve their goals and live fulfilling lives.
  • Employ Advantage Thinking practice principles to guide young people towards independence.
  • Meet contract, performance, and practice requirements, upholding ethical and professional standards.
  • Contribute to practice improvements in Aftercare, promoting high standards of service delivery.
  • Foster a culture that values young people's autonomy and self-determination.
Requirements
  • Diploma qualifications in community services and/or related field, and/or demonstrated relevant work experience.
  • Three or more years of experience working with young people.
  • Knowledge or willingness to learn Advantage Thinking practice model, coaching strategies, motivational interviewing, solution-focused questioning, and trauma-informed care.
  • Understanding of the complex needs of young people leaving care.
  • Ability to develop person-centered case plans and conduct coaching conversations.
  • Commitment to creating an environment where young people's voices are heard and valued.
