Job Overview
  • Lead Client Support Specialist Role

We are seeking a dedicated professional to oversee a designated caseload, handling all aspects from initial referral to intake and eventual exit.

Key Responsibilities:
  1. Engage in daily face-to-face client meetings, promptly addressing emergencies and ensuring positive outcomes through follow-up actions.
  2. Develop collaborative support plans with residents, incorporating interventions tailored to their needs and goals.
  3. Proactively gather feedback from support partners and conduct weekly assessments to devise additional strategies for enhanced client support.
  4. Monitor progress and outcomes of individual case plans, ensuring alignment with client needs and goals, including coordinating timely reviews and exit planning.
  5. Engage effectively with internal departments and Housing Services on complex residents, identifying collaborative strategies to sustain tenancies.
  6. Promote the Support Services and associated programs both internally and externally.
  7. Maintain up-to-date documentation and files within a timely manner in database systems and other data capturing tools.
Requirements:
  1. Relevant qualifications and experience in social work or a related field.
  2. Excellent communication and interpersonal skills.
  3. Ability to work collaboratively with diverse stakeholders.
  4. Strong problem-solving and analytical skills.
  5. Capacity to maintain confidentiality and handle sensitive information.
