- Lead Client Support Specialist Role
We are seeking a dedicated professional to oversee a designated caseload, handling all aspects from initial referral to intake and eventual exit.
Key Responsibilities:- Engage in daily face-to-face client meetings, promptly addressing emergencies and ensuring positive outcomes through follow-up actions.
- Develop collaborative support plans with residents, incorporating interventions tailored to their needs and goals.
- Proactively gather feedback from support partners and conduct weekly assessments to devise additional strategies for enhanced client support.
- Monitor progress and outcomes of individual case plans, ensuring alignment with client needs and goals, including coordinating timely reviews and exit planning.
- Engage effectively with internal departments and Housing Services on complex residents, identifying collaborative strategies to sustain tenancies.
- Promote the Support Services and associated programs both internally and externally.
- Maintain up-to-date documentation and files within a timely manner in database systems and other data capturing tools.
- Relevant qualifications and experience in social work or a related field.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with diverse stakeholders.
- Strong problem-solving and analytical skills.
- Capacity to maintain confidentiality and handle sensitive information.
