About the Role

As a Clinical Care Coordinator, you will report to the Clinical Care Manager and work collaboratively to advance positive health outcomes for our residents.

Key Responsibilities:
  • Conduct admission and ongoing care planning processes to ensure optimal resident care.
  • Promote effective case conferencing processes to enhance collaboration among healthcare professionals.
  • Participate in leadership teams to drive strategic planning and operational improvements.
  • Demonstrate expertise in resident health conditions to inform care decisions.
Requirements:
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Minimum three years' experience in aged care, with a strong focus on delivering high-quality patient care.
  • Current NDIS Workers Screening Check and Covid-19 vaccinations.
Preferred Qualifications:
  • Experience in re-accreditation processes and assessment visits through the Australian Aged Care Quality and Safety Commission.
  • Broad knowledge of the Aged Care Industry, including the Aged Care Act, 1997 and subordinate principles.
  • Strong understanding of clinical governance and accountability principles.
  • Excellent computer literacy and competency in electronic care systems.
