About the Role

As a Palliative Care Specialist, you will be part of a multidisciplinary team providing patient-centered, evidence-based care to individuals with palliative and end-of-life needs. You will work closely with the Grampians Regional Palliative Team and other healthcare providers to ensure seamless care coordination and access to specialist services.

Our team is committed to delivering high-quality care that is tailored to the individual needs of our patients and their families. We prioritize patient comfort, dignity, and quality of life, and strive to provide a supportive and compassionate environment for all.

About You
  • Recent experience and postgraduate qualifications in Palliative Care nursing or a related field.
  • At least 3 years' experience in Palliative Care, Community Nursing, or a related area.
  • A demonstrated commitment to delivering client-centered care that prioritizes the individual's needs and preferences.
  • A commitment to working collaboratively as part of a multidisciplinary team to achieve optimal patient outcomes.
  • The ability to assess, plan, and coordinate care for patients with complex medical, psychological, and chronic illness needs.
  • Excellent written and communication skills, with the ability to work effectively with patients, families, and healthcare teams.
  • A current and valid Victorian driver's license.
  • Current registration with AHPRA as a Registered Nurse.
