St Vincent's Health Australia is seeking a highly skilled Palliative Care Specialist to join our team at St Vincent's Hospital Melbourne.
About the Role
The successful candidate will contribute to the successful operation of the Palliative Care Consultancy Service (PCCS), providing specialist palliative care advice and consultancy to clinicians, patients, and families within SVHM, St George's Hospital, St Vincent's Private Hospital, and other affiliate organisations.
Key Responsibilities
- Assess and review patient and family care needs, symptom management, and future care planning
- Liaise with community palliative care services in Victoria and other stakeholders
- Contribute to discharge planning and educate and mentor clinicians throughout SVHM
- Develop and maintain own clinical development and competence, in line with best practice standards and NMBA regulations
Requirements
- Registered Nurse Practitioner with the Nursing and Midwifery Board of Australia and AHPRA registered
- Demonstrated experience practicing at an advanced clinical practice level in the area of palliative care
- Demonstrated ability to work with a multidisciplinary team and work both autonomously and collaboratively
