At Northeast Health Wangaratta, we are seeking a skilled Palliative Care Specialist to join our team. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality supportive care services to our community palliative care clients in their homes across the Rural City of Wangaratta and Alpine Shires.
Key Responsibilities:
- Plan, implement, evaluate, and deliver supportive care services to clients in their homes.
- Provide Palliative Care Volunteer Coordination, including recruitment, training, monitoring, support, and matching of volunteers to clients.
- Act as a clinical resource for staff and undertake supervision of relevant discipline staff and students.
- Continuously update your skills and knowledge relevant to Palliative Care and provide high-quality discipline-specific services, including assessment and interventions to clients/carers, peers, and the multidisciplinary team.
