Role Summary

We are seeking a skilled and compassionate Paediatric Social Worker to join our dynamic team. As a key member of the community health directorate, you will provide high-quality service across various settings, working collaboratively with multidisciplinary teams to promote an integrated and holistic service.

This role will involve supporting children and their families to access essential services, including the Infant, Child and Family Health and Wellbeing Hub. You will work closely with colleagues from the Child Family Program and Early Intervention Mental Health Service to deliver patient-centred care and advocate for the needs of patients.

Main Responsibilities:

  • To provide psychosocial support and advocacy through assessment, counselling, information provision, access to community resources and discharge planning.
  • To work collaboratively with the multidisciplinary team to deliver patient-centred care and advocate for patients' needs.
  • To support children and their families to access the Infant, Child and Family Health and Wellbeing Hub and other services as needed.

Requirements:

  • Bachelor of Social Work or Master of Social Work Qualifying OR Master of Social Work, OR equivalent as assessed and approved by the Australian Association of Social Workers (AASW).
  • Written evidence of eligibility for AASW membership for overseas qualifications is required.
  • Accredited Social Worker and/or Accredited Mental Health Social Worker: As approved by the AASW.
  • Working With Children's Check.
  • Police Check.
  • Driving Licence.
  • Australian working rights.
