Care Management Specialist

As a key member of our team, you will play a vital role in enhancing health outcomes for patients through population-based care management. This position requires extensive experience in obstetrics and gynecology care and a profound understanding of nursing processes.

Key Responsibilities:
  • Assess, plan, educate, monitor, evaluate, and coordinate services for patient populations and their families
  • Facilitate the coordinated utilization of resources to maximize health outcomes, patient/family satisfaction, and financial outcomes
  • Support patients, families, and caregivers in securing necessary medications, equipment, and community resources to facilitate health and overall wellness

Requirements include a Bachelor of Science in Nursing (BSN) or equivalent degree, current licensure to practice registered nursing in Ohio, and three years of clinical experience in care coordination.

Preferred Qualifications:
  • One year of Care Management experience
  • Knowledge of electronic medical records software
  • Additional related previous experience and/or preparation required for specialty care management

This is a full-time position with competitive pay and benefits. The salary range is $33.66 - $50.49 per hour.
