Job Opportunity: Community Based Nurse

We are seeking a dedicated and passionate nurse to deliver patient-centred care, supporting health, well-being, and quality of life for individuals at risk of hospitalisation.

  • This role involves assessing patients with chronic conditions, providing guidance on treatment options, symptom management, and lifestyle modifications.
  • The successful candidate will be skilled in wound assessment, tracheostomy care, enteral feeding management, and patient monitoring.
Key Responsibilities:
  • Deliver high-quality nursing care to support the health and well-being of clients.
  • Assess patients with complex needs, providing expert guidance on treatment options and symptom management.
Requirements:
  • AHPRA Registration - Current registration with the Australian Health Practitioner Regulation Agency is required.
  • Relevant Qualifications - A Nursing and/or Midwifery qualification is necessary for this role.
  • Vaccination Compliance - The ability to provide evidence of up-to-date vaccinations is mandatory.
  • National Police Certificate - A current National Police Check is required.
  • First Aid and CPR Certificate - Valid certification is essential.
  • Australian Driver's Licence & Reliable Transport - The ability to travel between client locations is necessary.
